I need to exchange/refund my order, how do I do that?

No!  That stinks.

First up, what's the reason for the exchange/refund?

Each shop on Mindful Market is independent and sets their own policies. Some shops will happily accept exchanges or refunds, no matter the reason. Other shops have a strict policy that all sales are final. You can look this up on the shop's FAQ page or contact the shop to find out.
